Infantile and Early-Onset Scoliosis: What Parents Need to Know
Scoliosis is a complex condition that can present at any age—but when it is diagnosed before the age of 10, it is classified as early-onset scoliosis. For children diagnosed before the age of 3, the condition is further defined as infantile scoliosis, a rare but important subset that requires careful attention, even if symptoms are not immediately severe.

Infantile scoliosis typically appears in children under 3 years of age. In many cases—particularly those with mild curves under 20 degrees and no other developmental abnormalities—the curvature may resolve on its own by the time the child turns three.
However, not all cases improve without treatment. Some children experience progressive spinal curvature, which can lead to significant issues in adolescence and adulthood if left unmanaged. That’s why early detection and close monitoring are essential.
Unlike most other forms of scoliosis, infantile scoliosis is more commonly diagnosed in boys than girls. It may occur independently or as part of a broader set of congenital abnormalities affecting other parts of the body. For this reason, a full diagnostic review—including imaging and neurological assessment—is recommended for all infants showing signs of spinal curvature.
For infants with curves greater than 20 degrees, MRI imaging is often advised to rule out other conditions such as:
* Congenital vertebral malformations
* Spinal cord tethering
* Arnold Chiari malformations
* Plagiocephaly or torticollis

The CLEAR™ Scoliosis Treatment protocols are primarily designed for children over the age of three who can actively participate in the program’s therapeutic exercises.
However, modifications can be made for very young children in certain cases to address:
* Cervical misalignments (such as torticollis)
* Cranial imbalances (e.g., plagiocephaly)
* Spinal tension due to neurological abnormalities
In these situations, CLEAR protocols may be used as a supportive therapy, not a stand-alone treatment. Our clinic may also co-manage care with pediatric orthopedic specialists when more comprehensive intervention is needed.
For progressive infantile scoliosis, the most widely recommended non-surgical treatment is EDF (Elongation, De-Rotation, Flexion) casting, also known as Mehta casting. This method is applied in a series of casts over time and has been shown to be highly effective in managing and even correcting curves in young children before the spine solidifies.
An important metric used in these cases is the Rib-Vertebral Angle Difference (RVAD), measured through x-rays. RVAD helps determine which spinal curves are likely to worsen and which are likely to resolve on their own.
Although chiropractic care is not a replacement for casting in progressive infantile scoliosis, it may be beneficial in supporting a child’s overall spinal health, especially when cervical or cranial alignment is affected.
